My four month old kitten has a neck injury that happened in a matter of minutes. Patient has prescription from the chiropractic to get the massage due to an auto accident. She has pain in the neck, trapezius and constant headache and a severe pain, need constant rate, soap note and all other information that I need to submit.
It’s my first time to prepare soap note to an insurance company.
